LIABILITY WAIVER・免責同意書

Kintsugi Pilates Studioのセッションに参加するにあたり、以下に同意します：

私は18歳以上であるか、または保護者の同意を得ています。
※18歳未満の方は、保護者が同意し、すべての責任を負うものとします。

セッションには運動負荷の高い動きや器具の使用（リフォーマー、キャデラック、チェアなど）が含まれることを理解しています。無理のない範囲で行い、必要に応じていつでも中止できます。

自身の健康と安全は自己責任で管理し、妊娠や持病、手術歴など必要な情報は事前に伝えています。

妊娠中または出産後の場合は医師の許可を得ています。

私は、Kintsugi Pilates Studio、スタッフ、関係者に対し、怪我や損失に関する一切の責任を免除します（重大な過失または故意の場合を除く）。

他者や設備への損害が生じた場合は、自分で責任を負います。

この同意書はすべてのセッションに適用され、必要に応じて法的手続きで使用される場合があります。

持ち物の管理は自己責任とし、紛失・盗難に対してスタジオは責任を負いません。

内容は予告なく変更される場合があります。

By participating in sessions at Kintsugi Pilates Studio, I confirm that:

I am 18 years or older, or have obtained parental consent.
For participants under 18, a parent or legal guardian must agree to this waiver and accept full responsibility.

I understand that Pilates may involve strenuous physical activity and equipment (e.g., Reformer, Cadillac, Chair, etc.). I am free to decline or stop at any time.

I take full responsibility for my own health and safety, and I have disclosed any relevant medical conditions, pregnancy, or recent surgeries.

If I am prenatal or postnatal, I confirm that I have received medical clearance.

I release Kintsugi Pilates Studio, its staff, and agents from any liability for injury, loss, or damage (except in cases of gross negligence or intentional misconduct).

I agree to be responsible for any injury or damage I may cause to others or to studio property.

I understand this waiver applies to all sessions and may be used in legal proceedings if necessary.

I understand personal belongings are my responsibility, and the studio is not liable for lost or stolen items.
