LIABILITY WAIVER・免責同意書

Kintsugi Pilates Studioのセッションに参加するにあたり、以下に同意します：

• 私は18歳以上であるか、または保護者の同意を得ています。

• セッションには運動負荷の高い動きや器具の使用（リフォーマー、キャデラック、チェアなど）が含まれることを理解しています。無理な動きは避け、いつでも中止できます。

• 自身の健康と安全は自己責任で管理し、妊娠や持病、手術歴など必要な情報は事前に伝えています。

• 妊娠中または出産後の場合は医師の許可を得ています。

• 私は、Kintsugi Pilates Studio、スタッフ、関係者に対し、怪我や損失に関する一切の責任を免除し、法的請求権を放棄します。

• 他者や設備への損害が生じた場合は、自分で責任を負います。

• この同意書はすべてのセッションに適用され、法的手続きにも効力を持つことを理解しています。

• 持ち物の管理は自己責任とし、紛失・盗難に対してスタジオは責任を負いません。

• 内容は予告なく変更される場合があります。

By participating in sessions at Kintsugi Pilates Studio, I confirm that:

• I am 18 years or older, or have obtained parental consent.

• I understand that Pilates may involve strenuous physical activity and equipment (e.g., Reformer, Cadillac, Chair, etc.). I am free to decline or stop at any time.

• I take full responsibility for my own health and safety, and I have disclosed any relevant medical conditions, pregnancy, or recent surgeries.

• If I am prenatal or postnatal, I confirm I have received medical clearance.

• I waive any legal claims and release Kintsugi Pilates Studio, its staff, and agents from any liability for injury, loss, or damage.

• I agree to be responsible for any injury or damage I may cause to others or the facility.

• I understand this waiver applies to all sessions and may be used in defense in legal proceedings.

• I understand personal belongings are my responsibility, and the studio is not liable for lost or stolen items.

• Terms are subject to change without notice.
